    ・Episode Summaries

‘The Madness of Taihō University Hospital’

Dr. Uesugi Saeko caused a medical accident and was dealing with the victim’s family. She’s invited to participate in a clinical trial for a new psychiatric drug, but it’s actually a conspiracy by the hospital.

‘The Madness of Taihō University Hospital: Night 2’

Yamaguchi Hiroko, wife of physician Yamaguchi Kōzō, is asked to bring her husband’s documents to the hospital. There she learns of an embezzlement case involving her husband. To prevent his arrest, Hiroko participates in a clinical drug trial prepared by the medical staff, only to face unimaginable humiliation.

‘The Madness of Taihō University Hospital: Night 3’

Physician Kosaka Nobuo encounters nurse Ōsaki Kaho being led around the lobby wearing a collar by Dr. Mutsuki Rieko from the psychiatric department. Rieko’s experiment involves publicly humiliating Kaho, whom she views as worthless—someone who causes trouble to family and others, gets them to pay off her debts, shows temporary remorse, then repeats the cycle.

    Editorial Review

Medical conspiracy visual novels occupy a peculiar niche in the adult doujin space, and this three-episode bundle from TKH Soft plants itself firmly in the intersection of psychological horror and SM themes—a combination rarely executed with sustained narrative commitment in English-accessible works. The Madness of Taihō University Hospital trilogy trades branching choice systems for linear storytelling, committing instead to atmosphere and unfolding dread across interconnected but standalone episodes.

What distinguishes this compilation is its willingness to marry psychological coercion with institutional horror rather than relying on convenience-store perversion. The married woman protagonist navigates a hospital environment thick with medical manipulation, restraint scenarios, and training dynamics that serve the narrative rather than interrupt it. The “rough” early-work aesthetics the publisher acknowledges actually benefit the horror positioning—clinical imprecision mirrors the disorienting power imbalances at the story’s core. Ten CGs per episode with variations is modest by contemporary doujin standards, but the constraint reinforces the visual novel’s focus on text-driven dread and character psychology over production spectacle.

The absence of voice acting and player choice creates an unusual experience: this is interactive fiction that trusts its scenario to carry weight rather than delegating narrative responsibility to player agency. For those fatigued by choice-based adult VNs, that commitment to authorial vision reads as refreshing rather than limiting. The interconnected structure invites multiple playthroughs precisely because discovery feels linear but complete within each episode.

Readers seeking narratively coherent psychological horror with adult content serving thematic purpose—where SM elements emerge from character dynamics rather than existing independently—will find genuine value here. Casual consumers expecting branching narratives or high production sheen should calibrate expectations accordingly. This is doujin work at its most deliberately specific.
